Predictive value of measuring carotid intima-media thickness by quality intima-media thickness technique in coronary heart disease

摘要: 目的:应用超声射频信号血管内-中膜分析(QIMT)技术测量颈总动脉内中膜厚度(CCA-IMT),探讨CCA-IMT对冠心病的预测价值。方法:根据冠状动脉造影将患者分为冠心病组和对照组,应用QIMT技术测量各组CCA-IMT。选用ROC曲线确定CCA-IMT预测冠心病的最适临界值。结果:对照组及冠心病组CCA-IMT分别为(772±71)μm及(1 043±91)μm,两组差异具有统计学意义(P<0.001);其中单支病变组和多支病变组CCA-IMT分别为(915±85)μm和(1124±70)μm,两组差异具有统计学意义(P<0.001)。ROC曲线确定的CCA-IMT临界值为850 μm,此临界值预测冠心病的灵敏度为91.7%,特异度为82.1%,准确度为72.6%;CCA-IMT>1 100 μm可作为冠心病诊断的参考指标。结论:CCA-IMT增厚预测冠心病的最适阈值为850 μm(0.85 mm),增厚大于1 100 μm(1.1 mm)时可作为诊断冠心病的参考指标。

Abstract: Objective:To investigate the predictive value of carotid intima-media thickness (CCA-IMT) in coronary heart disease (CHD) patients by using the quality intima-media thickness technique (QIMT) to measure and analyze CCA-IMT. Method:The patients were divided into control group and CHD group by the results of coronary angiography, and the CCA-IMT were measured by QIMT technique. Receiver operator curve (ROC curve) was used to determine the optimal threshold of CCA-IMT for predicting CHD. Result:The IMT value of patients in control group and CHD group were (772±71)μm and (1 043±91)μm, respectively (P<0.001). The IMT value in single and multiple vessel group were (915±85)μm and (1124±70)μm, respectively(P<0.001). From ROC curve analysis, the optimal threshold of CCA-IMT was determine at 850 μm,while the sensitivity was 91.7%, specificity was 82.1%, accuracy was 72.6%. CCA-IMT>1 100μm could be taken as the reference index in diagnosis of CHD.Conclusion:The optimal threshold of CCA-IMT thickening for predicting CHD is 850 μm (0.85 mm) and thickening, greater than 1 100μm (1.1 mm), can be taken as the reference index in diagnosis of CHD. -
