Relationship between the burden of functional premature ventricular contractions and the left cardiac structureand function

摘要: 目的:探讨功能性室性期前收缩(PVC)与患者心脏结构及功能之间的相关性。方法:采用回顾性分析对74例无器质性心脏病伴有频发PVC患者,根据PVC负荷分为低负荷组、中负荷组和高负荷组,以QRS波宽150ms为界值分为QRS<150ms组与QRS≥150ms组,比较左房内径(LA)、左室舒张末期内径(LVEDD)、左室收缩末期内径(LVESD)、左室射血分数(LVEF)及左室短轴缩短分数(FS)、左室舒张早期流速峰值(E峰)与二尖瓣环的最大运动速度(Em)的比值(E/Em)。结果:PVC负荷、QRS波宽与LA、E/Em无明确相关(P>0.05),随着PVC负荷增加、QRS波群增宽LVEDD、LVESD增大,LVEF、FS下降,差异有统计学意义(P<0.05)。直线相关性分析显示:PVC负荷与LVEDD、LVESD存在正相关(r=0.674;r=0.679),与LVEF、FS存在负相关(r=-0.406;r=-0.36),差异有统计学意义(P<0.05);QRS波宽与LVEDD、LVESD存在正相关(r=0.344;r=0.446),与LVEF、FS存在负相关(r=-0.413;r=-0.405),差异有统计学意义(P<0.05)。结论:功能性PVC的PVC负荷、QRS波宽与左室内径、左室收缩功能密切相关。PVC负荷高、QRS波群宽,易出现左心室扩大,左心收缩功能下降。Abstract: Objective: The aim of this study was to evaluate the relationship between the burden of functional premature ventricular contractions (PVC) and the cardiac structure and function.Method: A retrospective analysis of seventy-four patients without organic heart disease,but associated with frequent PVC were divided into 3groups (low burden group,middle burden group and high burden group) based on PVC burden.Patients were also divided into QRS < 150 ms group and QRS more than 150 ms group,according to QRS duration.We detected left atrial diameter (LA),left ventricular end diastolic diameter (LVEDD),left ventricular end systolic diameter (LVESD),left ventricular ejection fraction (LVEF) and left ventricular fractional shortening (FS),left ventricular early diastolic peak flow velocity (E peak) and the maximum velocity of the mitral annulus (Em) ratio (E/Em).Result: There was no clear correlation of PVC burden,QRS duration in LA and E/Em (P>0.05).With the PVC burden,QRS duration increased,LVEDD,LVESD also increased,whereas LVEF and FS decreased.There was a statistically significant difference (P<0.05).There was a positive linear correlation (P<0.05) between PVC burden and LVEDD,LVESD ((r=0.674,r=0.679) and a negative correlation (P<0.05) between PVC burden and LVEF,FS (r=-0.406;r=-0.36);There was a positive linear correlation (P<0.05) between QRS duration and LVEDD,LVESD (r=0.344,r=0.446) and a negative correlation (P<0.05) between QRS duration and LVEF,FS (r=-0.413;r=-0.405).Conclusion: There is a close correlation between PVC burden,QRS duration and left ventricular inner diameter,left ventricular systolic function.It is suggested that PVC burden increase,QRS duration prolonged more likely to induce left ventricular enlargement and left ventricular dysfunction.
