Changes of serum GAL and AgRP in type 2 diabetes and different classification of hypertension and its clinical value

摘要: 目的:探讨血清甘丙肽 (GAL) 和刺鼠基因相关蛋白 (AgRP) 水平在不同分级血压2型糖尿病患者中的改变及其与血糖、血压相关参数的关系。方法:纳入2型糖尿病患者197例及健康体检者 (NC组) 40名, 依据不同血压分级将糖尿病患者分为单纯糖尿病组 (DS组)、2型糖尿病合并高血压1级 (DH1组)、2型糖尿病合并高血压2级 (DH2组)、2型糖尿病合并高血压3级 (DH3组), 收集研究对象的一般资料并进行生化指标检测, 应用ELISA法检测研究对象血清GAL、AgRP水平。多元线性回归法分析GAL和AgRP的影响因素。结果:NC组的GAL水平较DS组减低, 在DH1组、DH2组和DH3组3组中, GAL水平随着血压的升高而降低, 且均低于DS组 (均P<0.05)。DS组、DH1组、DH2组和DH3组AgRP水平较NC组减低, 其中DH3组中AgRP水平最低 (均P<0.05)。Pearson相关分析提示:血清GAL水平与收缩压 (SBP)、舒张压 (DBP)、高血压病程 (HP病程)、体质指数 (BMI)、三酰甘油 (TG) 呈负相关, 与AgRP呈正相关 (P<0.05);血清AgRP水平与SBP、DBP、糖尿病病程 (DM病程)、HP病程、TG、空腹血糖 (FBG)、糖化血红蛋白 (HbA1c) 呈负相关, 与GAL呈正相关 (P<0.05)。进一步多元回归分析提示, SBP是GAL水平的重要影响因素, 同时也是血清AgRP水平的重要影响因素。结论:不同分级血压的2型糖尿病患者血清GAL和AgRP水平均减低, GAL和AgRP水平的变化对2型糖尿病和高血压的发生均有一定影响。Abstract: Objective: To investigate the levels of serum GAL and AgRP, and its relationship with blood pressure, blood glucose and related parameters in diabetes with different classification of hypertension.Method: One hundred and ninety-seven patients with type 2 diabetes and 40 normal control subjects (NC group) were enrolled in this study and the diabetes patients were divided into four group including simple diabetes (DS group), diabetes and grade 1 hypertension patients (DH1 group), diabetes and grade 2 hypertension patients (DH2 group), and diabetes and grade 3 hypertension patients (DH3 group).The general data of the subjects were collected and the biochemical indexes were detected.The levels of serum GAL and AgRP were detected by ELISA.The related factors of GAL and AgRP were analyzed by multiple linear regression analysis.Result: The level of GAL in DS group was higher than that in NC group, in the meantime, with the increase of blood pressure, the levels of GAL in DH1 group, DH2 group and DH3 group were gradually reduced (P<0.05).Compared with NC group, the levels of AgRP in DS group, DH1 group, DH2 group and DH3 group were decreased, and the lowest was DH3 group (P<0.05).Pearson correlation analysis reminded that GAL was negatively correlated with SBP, DBP, HP duration, BMI, and TG, as well as positively correlated with AgRP, meanwhile, AgRP was negatively correlated with SBP, DBP, DM duration, HP duration, TG, FBG and HbA1 C (P<0.05). Multiple linear regression analysis showed that SBP was independently correlated with GAL level (P<0.05);SBP was independently correlated with serum AgRP level (P<0.05). Conclusion: Serum levels of GAL and AgRP decreased in diabetes with different classification of hypertension.Both GAL and AgRP might closely be related to the occurrence and development of diabetes and hypertension.
