Expression of myocardial miR-138 in patients undergoing cardiopulmonary bypass surgery

摘要: 目的:检测体外循环手术患者心肌中miR-138的表达变化, 并探讨其与心肌损伤、炎症反应及临床预后的关系。方法:纳入于2015-06-2017-12在我科行室缺修补术+右室流出道疏通术的成年患者。采集右室流出道心肌标本, qRT-PCR检测miR-138表达。血液标本检测心肌标志物、B型钠尿肽 (BNP) 、C反应蛋白 (CRP) 、乳酸及白细胞介素 (IL) -6、IL-8的表达。结果:共纳入29例患者。术后心肌肌钙蛋白I (cTnI) 、肌酸激酶同工酶 (CK-MB) 、肌红蛋白 (Mb) 、BNP、CRP的表达均与miR-138呈显著负相关 (P<0.05) 。术后乳酸水平仅在第1小时与miR-138的表达有显著的直线相关关系 (P<0.05) 。术后第1天IL-6及IL-8的变化较大, 与miR-138的表达无相关性。患者心肌中miR-138的表达高低并不能显著改善患者的机械通气时间、ICU停留时间、总住院日。结论:miR-138与体外循环术后心肌损伤密切相关, 可作为潜在的新型心肌标志物, 本研究为临床早期心肌损伤检测和评估提供了新的策略。Abstract: Objective:The present study intends to detect the expression of myocardial miR-138 in patients undergoing cardiopulmonary surgery and explore its relationship with myocardial injury, inflammatory response and clinical prognosis.Method:Patients who have undergone ventricular septal defect repair combined with right ventricular outflow tract dredging surgery in our department from June 2015 to December 2017 were included in the present research.Myocardial samples from right ventricular outflow tract were collected to detect the expression of miR-138 by using qRT-PCR method.And myocardial biomarkers, BNP, CRP, lactic acid as well as IL-6, IL-8 in the blood were also measured.Result:A total of 29 patients were enrolled.Postoperative expression of cTnI, CK-MB, MB, BNP, CRP was significantly negatively correlated with miR-138 (P<0.05) .Only at postoperative 1 h, there was a significant linear correlation between the lactic acid level and the expression of miR-138 (P<0.05) .Expression of IL-6 and IL-8 varied a lot at postoperative 1 d, and there was no correlation with the expression of miR-138.The expression level of miR-138 in the myocardium of patients could not significantly improve the mechanical ventilation time, ICU stay time and total hospital stay of the patients.Conclusion:miR-138 is closely related to myocardial injury after cardiopulmonary bypass surgeries, and can be used as a potential new myocardial marker.This study provides a new strategy for early detection and evaluation of myocardial injury.
