A comparative study of electrophysiological examination and high frequency stimulation in left atria in patients with vasovagal syncope

摘要: 目的:对比研究血管迷走性晕厥患者常规电生理检查及左房高频刺激。方法:11例拟行迷走神经节消融的血管迷走型晕厥患者术中先行常规心脏电生理检查评价窦房结及房室结功能后,房间隔穿刺后,在左房迷走神经节丰富的部位行高频刺激方案,治疗终点:消融后不再出现心脏迷走反应。结果:11例常规电生理检查正常范围的患者,在左房高频刺激方案中3例检出显著心动过缓(P<0.05)。结论:在迷走型晕厥缓慢性心律失常排查中常规电生理检查有一定的漏诊,左房高频刺激方案可以作为补充,并为进一步迷走神经节消融提供指导。Abstract: Objective:To Compare the conventional electrophysiological examination and left atrial high frequency stimulation in patients with vasovagal syncope.Method:Eleven patients with vasovagal syncope scheduled to undergo vasovagal ganglion plexuses ablation underwent conventional electrophysiological examination to evaluate the function of sinoatrial node and atrioventricular node.High frequency stimulation was performed at the abundant sites of left atrial vagal ganglion after atrial septum puncture.Terminal therapy point:No cardiac vagal reaction occurred after ablation.Result:In 11 patients with normal range of routine electrophysiological examination,3 patients with significant bradycardia were detected in the high frequency stimulation(P<0.05).Conclusion:Conventional electrophysiological examinations in the screening of bradyarrhythmia in vagal syncope have some missed diagnosis.High frequency stimulation measurement can be used as a supplement and provide guidance for further vagal ganglion plexuses ablation.
