摘要: 目的:对小剂量多巴胺在急性心力衰竭(心衰)患者中应用的临床效果进行系统评估。方法:使用英文检索PubMed、Cochrane及Embase数据库,使用中文检索中国知网数据库、中国生物文献数据库及万方数据库。以急性心衰患者为研究对象,收集使用小剂量多巴胺和不使用多巴胺的随机对照临床研究,采用RevMan 5.3软件进行Meta分析。结果:本文纳入9篇RCT研究,共804例患者,小剂量多巴胺组364例,对照组440例。结果显示小剂量多巴胺能显著改善急性心衰患者的血肌酐水平及估测肾小球滤过率;但不能改善病死率、尿量、体重及住院时间。亚组分析结果显示,小剂量多巴胺能显著增加射血分数保留心衰(HFpEF)患者的尿量,降低HFpEF患者血肌酐水平。结论:小剂量多巴胺能保护急性心衰患者的肾脏功能,增加HFpEF患者的尿量,但不能减少心衰患者的病死率。Abstract: Objective: To evaluate the clinical effects of low-dose dopamine treatment in patients with acute decompensated heart failure(ADHF).Methods: Relative studies were identified in the Medline, Embase, and Cochrane databases. The articles in the China National Knowledge Internet(CNKI), the Chinese Biomedical Literature Database(CBM) and Wanfang Med Online were also searched using Chinese language. The randomized controlled trials(RCTs) that compared low-dose dopamine treatment with control treatment in patients with ADHF were included. Statistical analysis was performed using Review Manager 5.0.Results: Nine articles were included. In total, 804 patients were enrolled, 364 in the dopamine group and 440 in the diuretics group. The serum creatinine was decreased and the eGFR was increased in the low-dose dopamine group as compared with the control group. However, the mortality, urine output, weight loss and length of stay in hospital showed no significant differences between two groups. Subgroup analysis found that low-dose dopamine could increase urine volume and decrease serum creatinine in heart failure with preserved ejection fraction(HFpEF).Conclusion: Low-dose dopamine strategy could protect the renal function in patients with ADHF, and may be associated with increased urine volume in HFpEF subgroup.
