Predictive value of total bilirubin,fibrinogen/albumin ratio,and their combination in stent restenosis after PCI

摘要: 目的:探讨血清总胆红素(TBil)、纤维蛋白原(Fib)/白蛋白(Alb)的比值(FAR)及二者联合检测预测PCI术后支架内再狭窄(ISR)的应用价值。方法:选取2018年10月—2020年10月郑州大学第一附属医院心内科收治的接受PCI术并于术后1年复查冠状动脉造影的患者200例。根据复查造影结果,将患者分为ISR组和非ISR组(NISR组)。比较两组PCI术前一般临床资料及包括TBil、Fib、Alb在内的常规生化指标,采用多因素Logistic回归模型分析ISR的影响因素,并应用ROC曲线分析相关因素的预测价值。结果:ISR组糖尿病患病率、Fib、FAR水平高于NISR组,估测肾小球滤过率(eGFR)、红细胞数(RBC)、TBil、直接胆红素(DBil)、间接胆红素(IBil)水平低于NISR组(均P<0.05)。回归分析发现,TBil、FAR、eGFR、糖尿病史是ISR的独立影响因素(均P<0.05),其中FAR、糖尿病史为ISR的危险因素,而TBil、eGFR为保护因素。ROC曲线分析发现,TBil、FAR、TBil联合FAR、eGFR、糖尿病史对于ISR均有较高的预测价值(均P<0.05)。其中,TBil联合FAR的AUC高于TBil、FAR单一检测(AUC分别为0.788、0.728、0.736,P<0.05)。结论:TBil、FAR、eGFR、糖尿病史均是ISR的独立影响因素。其中,TBil、eGFR为保护因素,FAR、糖尿病史为危险因素。TBil联合FAR对于ISR的预测价值高于二者单独的预测价值。

Abstract: Objective: To explore the predictive values of serum total bilirubin(TBil), fibrinogen(Fib)/albumin(Alb) ratio(FAR), and their combination in stent restenosis(ISR) after PCI.Methods: A total of 200 patients who received PCI and were re-examined for coronary angiography one year later in the Department of Cardiology, the First Affiliated Hospital of Zhengzhou University from October 2018 to October 2020 were selected. According to the results of re-examination angiography, all patients were divided into stent restenosis group(ISR group) and non-stent restenosis group(NISR group). According to the results of re-examination and angiography, they were divided into ISR group and non-ISR group(NISR group). The general clinical data and routine biochemical indicators including TBil, Fib, Alb before PCI were compared between the two groups. The multivariate Logistic regression model was used to analyze the factors affecting ISR. The receiver operating curve(ROC curve) was used to analyze the predictive values of related factors.Results: Compared with the NISR group, the prevalence of diabetes, Fib, and FAR levels were higher, while estimated glomerular filtration rate(eGFR), red blood cell count(RBC), TBil, direct bilirubin(DBil), and indirect bilirubin(IBil) levels were lower in the ISR group(all P<0.05). Regression analysis found that TBil, FAR, eGFR, and diabetes history were independent influencing factors of ISR(all P<0.05). FAR and diabetes history were risk factors for ISR, while eGFR and ISR were protective factors. ROC curve analysis showed that TBil, FAR, TBil combined with FAR, eGFR, and diabetes history had high predictive value for ISR. The AUC of TBil combined with FAR detection was higher than TBil or FAR alone(AUC were 0.788, 0.728 and 0.736, respectively, P<0.05).Conclusion: TBil, FAR, eGFR, and diabetes history are all independent influencing factors of ISR after PCI. Tbil and eGFR are protective factors, while FAR and diabetes history are risk factors. The predictive value of TBIL combined with FAR for ISR is higher than that of TBIL or FAR alone.-
