Prognostic implication of HDL-C in patients with ST-elevation myocardial infarction treated with primary percutaneous coronary intervention

摘要: 目的:探讨HDL-C水平对接受急诊经皮冠状动脉介入治疗(PCI)的急性ST段抬高性心肌梗死(STEMI)患者住院期间预后的影响。方法:回顾性分析352例接受急诊PCI的STEMI患者的临床资料,根据入院空腹HDL-C水平分成HDL-C降低组(HDL-C<1.04mmol/L,125例)和HDL-C正常组(HDL-C≥1.04mmol/L,227例)。比较2组住院期间病死率和联合心脏事件的发生情况。结果:与HDL-C正常组比较,HDL-C降低组的Killip分级≥Ⅱ级比例和既往糖尿病史比例增加,TC、LDL-C水平下降,TG水平增加(均P<0.05)。HDL-C降低组的住院期间病死率较HDL-C正常组增加(P<0.01),但2组联合心脏事件发生率差异无统计学意义。多因素Logistic回归分析显示,HDL-C是患者住院期间病死率的独立危险因素(P<0.01)。结论:对于接受急诊PCI的STEMI患者,HDL-C水平降低与住院期间病死率增加有关。HDL-C是接受急诊PCI的STEMI患者住院期间病死率的独立危险因素。Abstract: Objective: To explore the prognostic value of HDL-C in patients with ST-elevation myocardial infarction(STEMI) treated with primary percutaneous coronary intervention(PCI). Method: A total of 352 patients with STEMI treated with primary PCI were enrolled and divided into low HDL-C group(HDL-C<1.04 mmol/L) and normal HDL-C group(HDL-C ≥ 1.04 mmol/L).End points were death and composite of cardiac events during in-hospital.The relationship between HDL-C and the above end points was testified. Result: Compared with patients in normal HDL-C group, patients in low HDL-C group had higher incidence of diabetes and Killip class ≥ Ⅱ, lower TC and LDL-C, higher TG and in-hospital mortality(all P<0.05).In multi-variable analysis, lowHDL-C was associated with the risk of death after adjusting for other independent predictors of outcomes(P<0.01). Conclusion: Significantly higher in-hospital mortality is associated with low HDL-C.In patients with STEMI undergoing primary PCI, a baseline HDL-C is an important prognostic indicator of in-hospital mortality.
