Change of asymmetric dimethylarginine and insulin resistance in essential hypertention patients and the effect of olmesartan

摘要: 目的:观察高血压患者非对称性二甲基精氨酸 (ADMA) 与胰岛素抵抗 (IR) 的变化, 探讨奥美沙坦的干预作用。方法:选择原发性高血压患者85例, 采用稳态模式评估法 (HOMA) 评价IR, 将高血压患者分为伴IR组51例, 和无IR组34例。另选择22例健康体检者作为对照组。所有受试者均检测血糖、胰岛素、ADMA水平, 并评估IR。观察高血压患者经奥美沙坦治疗前后上述指标的变化。结果:①血清ADMA水平及IR指数在伴IR组中显著高于无IR组和对照组 (均P<0.01), 无IR组血清ADMA水平显著高于对照组 (P<0.05);②Pearson相关分析显示, 在高血压患者中:血清ADMA与IR指数显著相关 (r=0.388, P<0.01);③与治疗前相比, 奥美沙坦能显著降低血清ADMA水平 (P<0.05)。结论:①高血压合并IR患者ADMA水平显著升高, ADMA的异常增高可能与高血压发病有关;②ADMA升高与IR具有相关性;③奥美沙坦能降低血清ADMA水平, 且可能与改善IR有关。

Abstract: Objective: To observe the change of asymmetric dimethylargi-nine (ADMA) and insulin resistance (IR) in essential hypertention patients and explore the effect of olmesartan.Method: All 85patients with essential hypertension were recruited.Homeostasis model assessment-insulin resistance index (HOMA-IR) was applied to estimate insulin resistance.Fifty-one patients were classified as essential hypertention with IR group (IR group), and 34as without IR group (non-IR group).Another 22healthy individuals were selected as control group.Levels of fasting serum glucose, insulin and ADMA were measured.HOMA-IR was calculated as a measure of insulin resistance.The changes of indexes above were observed before and after treatment of olmesartan in essential hypertension.Result: ①Serum concentration of ADMA and HOMA-IR were significantly increased in EH with IR as compared with those in EH without IR and controls (P<0.01, P<0.01), the levels of ADMA in EH without IR were significantly higher than those in controls (P<0.05).②In patients with EH, Pearson correlation analysis shows that levels of ADMA were positively correlated with HOMA-IR (r=0.388, P<0.01).③Olmesartan can significantly decrease the level of ADMA as compared with those unintervened by Olmesartan (P<0.05).Conclusion: ①The level of serum ADMA significantly is higher in patients with essential hypertension complicated IR than in without IR, the abnormal increase of ADMA may be related to the pathogeny of EH.②The abnormal increase of ADMA is closely related to IR.③Olmesartan may decrease the level of ADMA and probably be related to the improvement of IR.-
