Expression and significance of heparanase and white blood cell in patients with acute coronary syndrome

摘要: 目的:研究急性冠状动脉综合征(ACS)患者外周血单核细胞中乙酰肝素酶(HPSE)、白细胞计数与冠状动脉(冠脉)病变严重程度的相关性,探讨两者在ACS发病中的临床意义。方法:选取2016-12-2017-11包头医学院第一附属医院心内科住院患者86例为研究对象,所有患者均行冠脉造影。将患者分为对照组(30例)、不稳定型心绞痛(UA)组(UA组,30例)及急性心肌梗死(AMI)组(AMI组,26例)。采用实时定量RT-PCR法检测患者外周血单核细胞中HPSE的表达水平,采用血细胞仪检测外周血白细胞计数及分类,以冠脉造影与SYNTAX评分评估冠脉病变程度。采用Logistic回归分析分析ACS发生的影响因素。结果:外周血单核细胞与HPSE表达水平在ACS患者中存在增高趋势。Logistic回归分析显示,单核细胞绝对值、中性粒细胞绝对值与ACS发生独立相关。结论:HPSE可能参与ACS发病,白细胞计数可能为ACS的高危因素。Abstract: Objective: To study the correlation between the severity of coronary artery lesions and heparanase (HPSE) as well as white blood cell in patients with acute coronary syndrome (ACS), and to explore their clinical significances in the pathogenesis of ACS.Method: Eighty-six patients in the Department of Cardiology of the First Affiliated Hospital of Baotou Medical College from December 2016 to November 2017 were selected.All patients underwent coronary angiography.All patients were divided into control group (30 cases), unstable angina pectoris group (UA group, 30 cases) and acute myocardial infarction group (AMI group, 26 cases).Real-time quantitative RT-PCR was used to detect the expression of HPSE in peripheral blood mononuclear cells.Leukocyte count and classification in peripheral blood were detected by blood cell analyzer.Coronary angiography and SYNTAX score were used to evaluate the degree of coronary artery lesion.Logistic regression analysis was used to analyze the influencing factors of ACS.Result: The expression of HPSE and monocytes in peripheral blood tended to increase in patients with ACS.Logistic regression analysis showed that the absolute values of monocytes and neutrophils were independently correlated with ACS.Conclusion: HPSE may be involved in the pathogenesis of ACS, and leukocyte count may be a high risk factor for ACS.
