秋水仙碱预防心脏手术及射频消融术后心房颤动的meta分析

Meta-analysis of colchicine for prevention of atrial fibrillation after cardiac surgery or radiofrequency ablation

  • 目的:评估秋水仙碱在预防心脏手术及射频消融术后心房颤动(房颤)的应用价值。方法:通过计算机检索Pubmed数据库、EMBASE数据库、Cochrane图书馆、中国生物医学文献数据库、CNKI数据库、万方中文科技期刊数据库(自建库起至2019年8月),搜集有关秋水仙碱应用于心脏手术及射频消融术的相关文章,评估秋水仙碱在降低术后房颤的临床应用价值。结果:纳入7篇随机对照研究,包含手术患者1779例,其中秋水仙碱组891例,对照组888例。在心脏外科手术患者中,秋水仙碱组发生房颤的风险明显低于对照组,差异有统计学意义(RR:0.68,95%CI:0.56~0.82,P<0.05),并且秋水仙碱组住院天数低于对照组,差异有统计学意义(MD=1.32,P<0.05);在房颤射频消融术中,使用秋水仙碱术后再发房颤的风险也明显低于对照组(RR:0.57,95%CI:0.43~0.78,P<0.05)。但是秋水仙碱组发生不良反应的风险明显高于对照组(RR=2.64,P<0.05)。结论:秋水仙碱能够有效地降低心脏外科手术及射频消融术后房颤的风险,具有广阔的应用前景。
