Relationship between serum apoA1 levels and left ventricular systolic function in patients with coronary artery disease

摘要: 目的:观察和分析冠心病患者血清载脂蛋白A1(apoA1)与左室收缩功能之间的关系。方法:纳入经冠状动脉造影明确诊断为冠心病的患者共计3402例,按照血清apoA1水平的三分位数将患者分为高中低3组,比较各组之间患者基线资料和生化指标的差异,以及心力衰竭亚型在各组之间的分布;Logistic回归分析评估血清apoA1水平与左室收缩功能之间的关系。结果:高apoA1组的左室收缩功能较低apoA1组更好[左室射血分数(LVEF)(63.63±9.23)%∶(59.62±11.33)%,P<0.001];高apoA1组的射血分数降低性心力衰竭(HFrEF)患者比例也显著降低(1.5%∶5.2%,P<0.001)。Logistic回归分析显示在调整所有基线差异后,血清apoA1水平是重度心功能不全的保护性因素(P<0.001)。结论:血清apoA1水平与冠心病患者左室收缩功能相关,是冠心病患者心功能不全的保护性预测因子。Abstract: Objective: To investigate and evaluate the relationship between serum apolipoprotein A1(apoA1) level and Left ventricular systolic function in patients with coronary artery disease.Methods: A total of 3402 patients who underwent coronary angiography were diagnosed as coronary artery disease, and then divided into three groups according the serum apoA1 level tertiles. Patients' baseline characteristics and biochemical parameters were recorded and analyzed. We further evaluated the relationship among the serum apoA1 and various heart failure subtypes and left ventricular systolic function.Results: The left ventricular systolic function in the highest apoA1 group was better than in the low apoA1 group[LVEF(63.63±9.23)%vs(59.62±11.33)%, P<0.001], and there was a significant positive correlation between the serum apoA1 level and LVEF(r=0.122, P<0.001); The patients in highest apoA1 group also had lower proportion of heart failure with reduced ejection fraction(1.5% vs 5.2%, P<0.001). Logistic regression analysis showed that after adjusting for all baseline differences, higher apoA1 level remained significantly negatively associated with worse left ventricular systolic function.Conclusion: Serum apoA1 level is significantly positively associated with LVEF in patients with coronary heart disease, and it may be an independent protective predictor of left ventricular systolic function in patients with coronary heart disease.
