摘要: 目的:探索血维生素D水平与高血压患者血压控制的相关性。方法:本研究为横断面研究设计,纳入1727例因高血压进行降压治疗的患者,测量患者血压和血维生素D水平,根据血维生素D水平将患者分为维生素D正常组(血维生素D 75~250 nmol/L,338例)、维生素D不足组(血维生素D 50~74 nmol/L,610例)和维生素D缺乏组(血维生素D<50 nmol/L,779例)。多因素线性回归分析和限制性立方样条函数回归分析血维生素D水平与用药后血压控制的相关性。结果:维生素D水平正常的高血压患者收缩压和舒张压显著低于维生素D不足和维生素D缺乏患者,多因素校正混杂因素后,维生素D不足组和维生素D缺乏组较维生素D正常组收缩压分别升高1.35 mmHg(1mmHg=0.133 kPa)和4.50 mmHg(趋势检验P<0.001),舒张压分别升高1.59 mmHg和4.34 mmHg(趋势检验P<0.001)。限制性立方样条函数图显示维生素D水平与高血压进行降压治疗患者的收缩压和舒张压呈线性负相关。结论:维生素D缺乏在中国人群中常见,血维生素D水平与高血压进行降压治疗患者的收缩压和舒张压呈线性负相关。Abstract: Objective: To explore the association between plasma vitamin D level and blood pressure control in patients with hypertension.Methods: This study was a cross-sectional design. A total of 1727 patients with hypertension prescribed antihypertensive drugs were enrolled. Blood pressure and plasma vitamin D levels were measured. According to plasma vitamin D levels, the patients were divided into vitamin D sufficiency group(vitamin D 75-250 nmol/L, n=338), vitamin D insufficiency group(vitamin D 50-74 nmol/L, n=610) and vitamin D deficiency group(vitamin D<50 nmol/L, n=779). Multivariate linear regression analysis and restrictive cubic spline regression models were performed to analyze the association between plasma vitamin D level and blood pressure control.Results: Systolic blood pressure and diastolic blood pressure in patients with vitamin D sufficiency group were significantly lower than those in patients with vitamin D insufficiency group and vitamin deficiency group. After adjusting for confounding factors, compared with patients with vitamin D sufficiency, systolic blood pressure in patients with vitamin D insufficiency and vitamin D deficiency increased by 1.35 mmHg and 4.50 mmHg respectively(P for trend<0.001), diastolic blood pressure increased by 1.59 mmHg and 4.34 mmHg respectively(P for trend<0.001). Restrictive cubic spline function showed that vitamin D level was linearly and negatively associated with systolic and diastolic blood pressure.Conclusion: Vitamin D deficiency is common in Chinese population, and plasma vitamin D level is linearly and negatively associated with systolic and diastolic blood pressure.
