Early changes of right ventricular longitudinal function of myocardial damage in alcoholics during subclinical stage by two-dimensional speckle tracking echocardiography

摘要: 目的:通过二维斑点追踪技术(2D-STE)评价亚临床期酒精性心肌损害右心室纵向功能的改变。方法:选取92例男性饮酒者分为3组:B组30例,每日饮啤酒2~3瓶(1 100~1 650 ml)或白酒≥ 90 g (即纯乙醇75 ml),每周3~5 d,时间5~8年;C组31例,饮酒量同B组,每周3~5 d,时间9~20年;D组31例,每日饮啤酒≥ 4瓶(≥ 2 200 ml)或白酒≥ 150 g (即纯乙醇125 ml),每周6~7 d,时间10年以上,饮酒量符合酒精性心肌病(ACM)诊断标准。另选取健康不饮酒且年龄相匹配的正常对照组(A组)30例。对4组进行常规超声心动图及2D-STE参数测量。检查指标收缩期参数(包括右心室游离壁基底段、中间段及心尖段收缩期纵向位移峰值,收缩期应变峰值,收缩期应变率峰值)和舒张期参数(包括舒张早期应变率峰值、舒张晚期应变率峰值)。结果:D组右室舒末内径(RVEDd)较A、B、C组显著增加(均P<0.05);与A组和B组相比,C组各节段舒张期纵向应变率舒张早期、晚期应变率峰值开始下降(均P<0.05),D组舒张期及收缩期所有参数均明显下降(均P<0.05);另外,C组与D组相比,舒张期参数差异无统计学意义,而D组收缩期纵向位移峰值、应变峰值则明显降低(均P<0.05)。结论:2D-STE可作为有效评价亚临床期酒精性心肌损害右心室纵向功能的一种无创性的新方法,对指导和判断预后具有重要的临床价值。Abstract: Objective: To characterize the early changes of right ventricular longitudinal function of myocardial damage in alcoholics during subclinical stage by two-dimensional speckle tracking echocardiography (2D-STE).Method: We classified 92 asymptomatic alcoholics into mild (group B), moderate (group C) and severe groups (group D), and 30 age-matched healthy males without drinking history were enrolled as control group (group A). Conventional echocardiography and 2D-STE parameters were obtained. 2D-STE images of apical four-chamber views were used for offline analyses of RV free wall, including peak systolic longitudinal displacement (D), strain (S), systolic strain rate (SRs), early diastolic strain rate (SRe) and late diastolic strain rate (SRa).Result: Compared with groups A, B and C, right ventricular end-diastolic diameter (RVEDd) significantly increased in group D (all P<0.05), whereas RVEF did not change significantly (P<0.05). 2D-STE parameters:we found no difference of all measured variables between group A and B(all P>0.05);compared with groups A and B, longitudinal SRe and SRa in group C of basal, mid and apical segments of RV free wall began to decrease(all P<0.05);all measured variables of group D decreased significantly(all P<0.05);moreover, there were no significant difference in SRe and SRa between group D and group C(all P<0.05), however, D, S, SRs significantly decreased in group D(all P<0.05).Conclusion: 2D-STE provides an effective method to assess the RV longitudinal function, which is important in diagnosing, directing treatment and judging prognosis of myocardial damage in Alcoholics during Subclinical stage.
