Correlation between plasma granule protein precursors and acute coronary syndrome

  • 目的:探讨血浆颗粒蛋白前体(PGRN)水平与急性冠状动脉综合征(ACS)的相关性,评价其对ACS各亚型[不稳定型心绞痛(UA)、非ST段抬高型心肌梗死(NSTEMI)、ST段抬高型心肌梗死(STEMI)]的诊断能力。方法:收集ACS患者150例(包括UA患者52例,NSTEMI患者51例,STEMI患者47例)。另选取冠状动脉造影阴性对照组41例。收集临床基线资料,分析ACS危险因素水平。通过酶联免疫吸附试验(ELISA)测定血浆PGRN水平,评价其与ACS各亚型的相关性。通过受试者工作特征曲线(ROC)评价PGRN对ACS亚型的诊断效能。结果:随着ACS病理程度(对照-UA-NSTEMI-STEMI)进展,PGRN水平逐渐下降[12.06(8.41,23.79)、7.91(5.86,13.46)、7.01(4.82,9.30)、3.45(2.26,5.71)ng/mL,P<0.001]。ROC曲线分析表明,PGRN水平鉴别诊断对照和STEMI的曲线下面积(AUC)为0.885(95%CI:0.815~0.954,P<0.001),灵敏度和特异度分别为0.88和0.81;PGRN鉴别UA和STEMI的AUC为0.823(95%CI:0.736~0.911,P<0.001),灵敏度和特异度为0.92和0.70。多元Logistic回归分析提示,PGRN水平是影响ACS发生的独立保护因素。结论:血浆PGRN水平与ACS发生相关,且有望作为诊断STEMI的新型标志物。
